Considerations in Complications Arising from the Management of Pemphigus Vulgaris: A Case Report

Background: This paper reports a case of pemphigus vulgaris in middle-aged male with oral lesions and skin eruptions
 Objective: To highlight the complications prolonged steroid therapy.
 Case Report
 A 48-year old presented an eight-month history recurrent mouth ulcers. There was positive cutaneous ulcers on upper extremities, back genital areas. Patient had been placed long term therapy prescribed by general practitioners. General examination showed bilateral pitting pedal oedema, moon face low blood pressure. Intra-oral revealed multiple irregular shaped erosions erythematous floor buccal mucosa, labial mouth, soft palate anterior pillar fauces. The dorsum tongue areas ulcerations necrotic slough. An impression made based high index clinical suspicion patient commenced azathioprine 50mg prednisolone 40mg daily for week. Topical use clobetasol ointment (0.05%) also to be applied twice daily. improvement one-week review. dose then stepped down 20mg one week 0.2% chlorhexidine rinse introduced.
 Punch biopsy intact mucosa done subsequent visit histopathology confirmed diagnosis vulgaris. referred cardiologist account hypotension oedema which suggestive long-standing use.
 maintenance 10mg daily, topical rinse.
 Conclusion: study male. developed some due duration therapy. Regular monitoring, adjustment combined adjuncts like steroid-sparing drugs are essential minimizing steroid-induced adverse effect
